If I understand correctly, in a SaaS environment, I cannot use a backup I download from the Backup Access Service to restore my production tenant. I need to pay separately to have Acumatica restore from backup?

Backup restore will be done the environment hosted company,
if it is hosted by Acumatica then you need submit a ticket to restore the DB in the required environment.

To add to my question - the SaaS environment contains a production tenant and a test tenant. Can the backup from the Backup Access Service be used to restore the test tenant?

I dont think there is a option to divide the DB in tenant wise, It is single BAK fle.
