The Washington state CPR form has been removed from the Government Reporting screen as the Aatrix Development team is currently making adjustments to the form. 

It will added back to the list by the end of the week, at the latest. 

If you are currently using Aatrix for filing this report - please submit a case with Aatrix. They are working with all those who are currently affected by the removal of the form and with processing any current or previously submitted filings.
