I created custom Inquiry form which provides Project Summary information and the Inquiry form working fine as I get desire result when I run directly but when I add new widget and try to select that screen from Inquiry screen drop down the created custom inquiry form is not available. Is there any way I can create widget from custom created inquiry form?

Assuming that by inquiry form you mean a Generic Inquiry you likely just need to make it visible on the UI. Then it should show up in the Inquiry Screen selector on the Widget Properties.
