Can another Shipping Destination Type be added for Purchase Orders? I’ve been asked to add “Custom” as another Destination Type but I don’t see where these are defined or where I can add a choice in the drop down list.

This would need to be customized to change the values.
You could create a customization project for that:

Yes, it would be.
You can always add an idea in the Community to have those field customizable easier. I know I’ve been asked several times if they could be changed.
