Inventory on appointment, not on invoice

I have a service order with an inventory item on an appointment, but the item did not flow through to the invoice.  I am not sure how this could happen.  Any thoughts?

Check the Service Order Type/Generated Billing Document; this cannot be AR Document.  Must be SO Invoice or Sales Order.

Is there a unit cost on the items and are the lines marked as Billable? 
Is this issue specific to one appointment or affecting multiple appointments? Any recent changes/updates that would affect the functionality?

Yes, there is a unit cost, and the lines are marked as billable.

Yes, the issue is specific to one appointment.  

No recent changes/updates that I am aware of that would have impacted functionality.
