"Object reference not set to an instance of an object" error appear whenever we tried to create a log from Staff Screen

"Object reference not set to an instance of an object" error will appear whenever we tried to create a log (press start) from Staff screen. And after that the tick button will appear but we couldn’t even select it to press save and we need to quit the screen and refresh.

Any advise or assistance on this?
