Appointments screen has two key fields Service Order Type and Appointment Nbr.
In your mapping you only have Appointment Nbr mapped but Service Order Type mapping line is missing.
Please try this:
* Key and Action Cancel lines are automatically added by the system once you map highlighted lines below.
