I have been looking out for ways to add an approval map whenever a new COA is added and then recording such approvals when, by whom the COA was added and by whom it was approved kind of audit report.

As per the standard design, there is no option to add an approval map to the Chart of Accounts screen.
To check the update history you can try the field level audit functionality.

Adding to YasasWithanage76’s recommendations, Here is an example of how to Audit Chart of Accounts screen, all fields:
Also consider creating a Generic Inquiry from Account table, and then use your Generic Inquiry as a basis for a Business Event, to monitor additions.
2024 r1 adds new screens for Approval Maps; Chart of Accounts is not one of the new screens, not yet included. I noticed Chart of Accounts is included in a proposed Idea in this community - see reply by Doug Johnson of Acumatica; please vote and add Chart of Accounts in a comment to this thread:
