Manufacturing Rework

Can someone suggest a good process for performing rework for a production order that is already in process.

For example, they decide that some additional insulation needs to be added to a part in an opeation that has already been completed.  This means that additional material and labor will need to be added and captured as rework material and labor.

I assume this is more complicated than putting the production order on hold and adding an operation to the production order w/ the necessary material / labor.  If so, it might be good to add more detail explaining your scenario.
