In Access Rights by Role screen , I find some screens displayed twice or thrice but not sure why ? What would be the reason ?

I would suspect because they are in the Site Map (SM200520)

Yes, I agree with dcomerford. That means that different forms/reports/gi were added to Site Map with the same title.
if you hover your cursor on the node of the form (tree on the left panel) then you should be able to see the number of the form the tile is associated with:
