Has anyone encountered error when trying to add a brand new template item?

I am using build 2022R2 build 22.206.0035 and now I get the following error every time I try to create a brand new template item.

Are there any customizations that may be effecting that screen in the instance?

I had this happen recently with a client where I could not export an excel file from a screen, it was due to a customization. Always worth a shot to look there first. 


It is definitely one of the customizations.  Now I just have to determine which one through fault isolation. :)
